The IRB has updated the short form consent template. In addition, the short form consent, both Adult and Minor versions, have now been translated into three languages for your use:
- Spanish (Latin American)
- Chinese (Basic)
- Arabic
Each form would require the addition of study title and PI contact information. Translation certificates are also available for each form.
If you require translation of the short form or the long form to another language (other than the three languages above), you will still be required to provide the forward translation with certification or forward and back-translation with attestation from the PI of bilingual skill.
