DUHS Consent Form Template Update

The DUHS IRB is pleased to announce an important update to our consent form templates following internal consideration and a comprehensive comparison with those utilized by peer institutions. As a result of this review, we have revised the templates to remove subject/parent/LAR initial lines that were deemed unnecessary. This update is aimed at streamlining processes and improving compliance for all stakeholders involved.  This change will be effective 7/1/2024.

The new template should be used for all new studies and amendments that include a new consent form.  The DUHS IRB will implement this change on consent forms during the next continuing review requiring form watermarking (e.g., open to enrollment, consents required for re-consent).

Study teams should not submit an amendment to remove initial lines.  In the interim, you may file a memorandum stating that initials on each page of the consent form will no longer be required from this date onward. This will serve as a temporary measure until the updated templates are fully integrated or for those studies that do not require continuing renewal.
