Subject Initials on Last Page of DUHS Consent Form Template

The IRB has removed the line for the subject’s initials on the final page of the consent form template.  Formerly, the subject was required to initial the bottom of each page of the consent form, but was never required to initial the signature page.  In order to avoid confusion, the line for the subject’s initials on the signature page has been removed.  The subject will provide signature, date and time on the last page, as usual. The IRB reminds you not to modify the information appearing in the footer of any DUHS consent form templates.  When developing a consent form for a new study, always get the current version of the DUHS consent form template from the IRB web site or the eIRB web site, and then you can edit the template to present the information specific to your study.
